Why do we need to process information about you?

In providing you with counselling services as Crabtree Therapy, we will need to obtain and handle your personal information, specifically:

 

Name and age:

This is basic information that helps us to get to know you.

 

Address, email address, telephone number: We use this as a way of contacting you regarding your sessions. We will mainly use the method that we have agreed at your assessment session.

 

Doctor’s details: If we are worried that you are at risk of serious harm then we may need to contact your doctor (please see the Therapeutic Agreement for more details).

This information is essential to inform, facilitate and provide assessment and therapeutic services which are appropriate to your individual needs.

 

Where and how do we keep this information?

The data on this paper and on our therapeutic agreement is kept in a locked filing cabinet. We transfer your name and unique code to our password-protected laptop and your telephone numbers may be kept on a mobile telephone which is code-protected. Client notes showing date and a brief outline of session content will be stored on our laptop.

 

We use a personal computer that only employees of Crabtree Therapy have access to. It is fully virus and password protected.

 

How long will Crabtree Therapy store this information for?

Crabtree Therapy will hold information about you for as long as you receive services from us and then for 7 years following the date of the last session. You have the Right to Erasure and can ask us to remove any personal information we hold about you. If you wish to evoke your Right to Erasure please email us at polly@crabtreetherapy.com. Please note that in some circumstances, e.g., commitment to HMRC inspection of our financial records, we may have to keep some personal information that we hold about you. In these circumstances, we will let you know as soon as possible and we will agree on a mutually acceptable date to erase.

 

How can you access the information Crabtree Therapy holds?

You have the Right to Access the personal information that Crabtree Therapy holds about you. If you wish to evoke your Right to Access please email us at polly@crabtreetherapy.com.

 

What if you believe the information Crabtree Therapy holds about you is incorrect?

Crabtree Therapy holds the personal information you have provided to us at the point of first contact. Please update us with any changes to your name or contact details. If you believe we hold inaccurate information then please email us your updates at polly@crabtreetherapy.com and we will update our records upon receipt.

 

Protecting your Information

Crabtree Therapy is committed to keeping the information we hold about you secure. To protect your personal data, we follow the guidelines and recommendations in line with professional and regulatory bodies such as the Information Commissioner's Office. In the unlikely event of our security processes being compromised leading to a significant breach of your information, we will endeavour to inform you within 72 hours.

 

Confidentiality

The confidentiality of your personal information is very important to Crabtree Therapy. All our services are confidential, and we will not share your information unless we judge that there is a serious risk of harm to yourself or others, or with your written consent, or when we are legally obliged to do so (please see the Therapeutic Agreement for more details).

 

Professional Will

In accordance with the requirements of the National Counselling and Psychotherapy Society, we are required to hold a professional will in the event that your therapist is unable to see you in the short term or in the event of your therapist’s death. Under these circumstances, you will be contacted by a professional colleague, Katy Baldock, who has a list of clients' first names and contact telephone numbers. She does not hold any clinical information about you.

Crabtree Therapy’s website, hosted on the Wix platform, may use cookies and similar tracking technologies to enhance user experience and analyse site traffic. Cookies are small data files that are placed on your device when you visit a website. You can control the use of cookies at the individual browser level.
